 TEMPORARY PASSPORTA temporary passport can be provided within 24 hrs in place of lost, stolen or damaged Polish passport. Required: name, date and place of birth, PESEL number, and a photo (can be digital). Fees: Money Order/Certified Check payable to Consulate of Poland in the amount of $58, and $40 consular fee (cash). |  VITAL RECORDSThe registration ("transcription") of vital statistics records (Birth, Marriage and Death Certificates) in Poland. |  CERTIFIED TRANSLATIONEnglish-Polish and Polish-English certified translation of all documents (DMV, vital statistics records, court, etc.). |

 NEVADA DMVProviding certified by Nevada DMV document translation as well as interpreting assistance with the written driver's license exam. |  NOTARY SERVICESNotarization of documents in English and Polish language. For documents to be used in Poland: 1. Notary Stamp will have stamp content translation attached; 2. obtaining Apostille from Secretary of State will be required, as well as 3.
